Since it's Elliott Smith, I guess this is played in D-Standard tuning
orginally,
but it works fine in Standard as well. Only on the D5/A are you missing that
low D from what I can hear.
There are some quite non-standard voicings, so I would really recommend
checking out the chord diagram below..

Most times the base note is hit once or twice before you strum the whole
Chord.
The Csus4 -> C5 Change has a hammer-on on the D-string.
And the D7 comes just before the change back to Gm.
It's much easier of you play the barred version of Cm/G,
but the one described above sounds more accurate to me.
